Moving can be a daunting experience.
Hope Pathway hopes to make this process as smooth as possible for all involved, ensuring young people are placed in the correct service and receive the appropriate level of care to meet their individual needs.
When considering referrals, Hope Pathway will always prioritize meeting both the needs of the young person referred and of those already in placement. We aim to respond to all referrals within a 24-hour period.
We will make every effort to appropriately match any young person to the most suitable placement. All referrals received by Hope Pathway’s semi-independent service will be managed in such a way as to ensure minimal disruption to the young person referred, the young people already in placement, staff and the general running of the home.
In order for us to make an informed decision, we ask that local authorities send a comprehensive referral to our home manager, highlighting the young person’s behaviors, needs and background history.
Our home manager will review the information as to whether a placement can be offered.
We will always be mindful of a young person’s cultural, religious, communication, and dietary requirements.
